Audits
There is no one taking responsibility for performing audits. It has become a perfunctory exercise on the path to competing for a specialty training post. There should be, in every hospital, a dedicated independent team and electronic database to continuously collect and audit clinical data. The results must be reviewed monthly. (7.66)
Consenting
Surgeons don’t ‘consent’ patients for the operation. It is the patient who ‘consents’ the surgeon to do the operation on their body. (6.12-13)
Leadership

Patients get seen by more than one surgeon in a team. Encourage a culture where individual practice may be available for scrutiny by the rest of the team. (7.44)
